This form must be completed if your child has a medical condition, allergy or intolerance. We are obliged to update this information every year, and all information is circulated to staff, many of whom may be new. Please do not assume that the schools know about your child's needs just because you have told us before.

If your child requires medication in school hours, please print out and complete the following document.  The document needs to be brought to school with the medicine for safe storage in the front office.

NB: please ensure that you complete the form for medication that is carried around by pupils such as inhalers and epipens as well.
